Wellness Plan
Have ever started a diet?
Have ever started an exercise program?
Have ever had a New Years Resolution involving your health?
What happened when you went on vacation, had a big
project at work or school, the kids were sick, life got busy, etc. the
diet and exercise routine just faded away and you were right back where
you started.
The goal of this plan is to make a series of small
changes that you incorporate into your lifestyle. That way when you
finish, you accomplish the same goal you intended, but in smaller more
manageable parts it is much easier to complete. A diet or exercise
program that remains a “program” but never become a part of your lifestyle
will fade when you get busy and it is next to impossible to maintain.
Reminders:
 | Changing a Lifestyle takes time |
 | Doing too much too fast leads to burn out and you
are more likely to stop |
 | Do it with a friend or group (motivation and
accountability) |
 | Some days, weeks, or months are harder try your
best during difficult time and get back on track as soon as possible.

 | Small changes add up (If you just improved 1% each
week and the end of a year you have experienced over 50% improvement.) |
 | Cheat every once in a while if you need to. If a
program is too rigid you will be less likely to follow through long
enough to make it part of your lifestyle. |
 | There is no such thing as a quick fix (pills,
diets, exercise programs, etc.) |
Let’s start by writing down your goals for your
Personal Wellness Plan. Be as specific as you can be.
Goals:
